All Job Related Injury insurances ie: Sedgwick, Carpenters Trust, Eberle Vivian etc… Please call if you have any questions

We are contracted with all Major Insurance companies. Please check the Insurance list and call with any questions you may have. Prior to your Initial Evaluation we will take your Insurance Information.

We will contact your insurance company to check and verify your benefits. This is something that you can do as well by contacting the Customer Service number found on your card.

 

Billing is done on a daily basis and 75% is completed electronically. Claims are processed usually within a 10 business day period. We bill our patients on a monthly basis for any responsibility they may have. We accept Mastercard, VISA, cash and check for payment.
